Chapter 41 Summary: “Briana”

Briana joins the bachelorette party but can’t stop thinking about Jacob. When one of the bridesmaids makes rude comments about him, Briana defends him and then goes to a convenience store to buy cigarettes and candy. Briana returns to Joy’s house and takes the cigarettes to Jacob’s grandpa. While he smokes, she eats candy. Amy comes out and confesses that everyone gives Grandpa cigarettes and that Joy knows, she just doesn’t enable him herself. Amy apologizes for her bridesmaid and confesses that she never took Jacob’s anxiety disorder seriously. Amy tells Briana that she has no doubt that Jacob loves her.

Briana goes to Jacob’s and tells him that she’s moving out. She accuses him of still loving Amy, and he accuses her of having feelings for Levi. The truth comes out, and they confess their love for each other.

Chapter 42 Summary: “Jacob”

On the day of Jeremiah and Amy’s wedding, Jacob’s family gathers at a hotel for the ceremony. Briana is amused to see Benny arrive as Jacob’s sister Jane’s date. Jacob and Briana annoy Jewel and Gwen with their constant public displays of affection.

Chapter 43 Summary: “Briana”

Briana and Jacob stay at the wedding reception until it ends and then kiss on the elevator ride to their room. They are so focused on each other that they don’t notice when the elevator stops to allow more people on. When Briana turns, she comes face-to-face with Nick and Kelly. They are wearing wedding rings, and Kelly is obviously pregnant. Briana feels ill.

Chapter 44 Summary: “Jacob”

Briana is silent on the ride back to Jacob’s. When they arrive, she announces that she’s moving home. When Jacob protests, Briana tells him how she learned Nick was cheating on her and that she miscarried shortly after he left. Jacob tries to convince her that he won’t do those things to her, but Briana insists she needs to be alone.

Chapter 45 Summary: “Briana”

Briana realized when she saw Kelly that she, too, is pregnant. She tells her mother that she took several tests as soon as she got home. Briana refuses to entertain the idea of marrying ever again, insisting that men all leave. Rosa tells her that some men leave, but not all. She talks about her second marriage and how happy it is. Although Briana knows Jacob is different, she can’t believe he won’t leave, so she feels that she needs to leave him first.

Chapter 46 Summary: “Jacob”

After a few days, Briana agrees to see Jacob. She tells him she’s pregnant but says that she can’t be in a relationship with him because she won’t be able to handle it if he ever leaves her. Jacob visits his mother and tells her everything that’s happening with Briana. Joy advises Jacob to continue showing up for Briana and encourage her to talk to a therapist.

Chapter 47 Summary: “Briana”

Alexis arrives at the hospital to be with Briana during Jacob’s and Benny’s surgeries. Briana goes to see Jacob, and he gives her his journal, asking her to read the marked passages. During the surgery, Briana sits in the chapel and reads Jacob’s journal, starting on the day they met. She reads his impressions of her and the emotions he felt, all the things he could never directly tell her. At the end of the journal, Jacob has written a letter to her, promising to “be harmless” to her. Briana decides to be brave.

Chapter 48 Summary: “Jacob”

Briana is with Jacob when he wakes up after surgery. She tells him she wants to move back in with him, and he suggests they get a house together that will be in both their names. Jacob promises to help Briana with trust the same way she helped him with his anxiety.

Epilogue Summary: “Jacob”

Two years later, Briana and Jacob prepare to get married at his cabin. Briana has attended counseling, and Jacob has become a stay-at-home dad to their daughter, Ava. Benny’s transplant went well, and he’s back at work and dating Jane. Jacob and Briana write their own vows in which they simply promise to “be harmless” to each other.

Chapter 41-Epilogue Analysis

The theme of First Do No Harm grounds these final chapters. Throughout the novel, Briana and Jacob have unintentionally hurt each other by failing to admit their true feelings. Even after they tell each other the truth and there are no more misunderstandings, their relationship faces challenges. Briana’s past trauma from her marriage and her father’s abandonment returns when she sees her ex-husband happy with his new wife. Although Jacob promises to do no harm to Briana, the harm has already been done. Briana doesn’t trust herself to know when someone is being truthful to her or not. Briana’s professed lack of trust in men throughout the novel is not about the men but about herself. She can’t believe that she can return to the same level of happiness and complacency and not experience hurt again.

Like Jacob’s anxiety, the trauma of Briana’s past is a part of her character that must be incorporated into their relationship. Briana adjusted easily to Jacob’s anxiety, but it is more difficult for Briana to allow Jacob to adjust to her lack of faith in love and commitment. The chance meeting with Nick and Kelly symbolizes a return to reality for Briana during her honeymoon period within her new relationship with Jacob. Not only this, but the realization that she is pregnant also reminds her that Nick hadn’t wanted her child; therefore Jacob might not want it, either.

The narrative comes full circle by returning to the written word through Jacob’s journals, echoing the letters that allowed Jacob and Briana to begin building an intimate relationship in the first place. The journal allows Briana to come to a new conclusion regarding Jacob in the time it takes him to donate his kidney to Benny. There is symbolism in the kidney donation as well, with Jacob giving up an organ to her brother while Briana decides to take a leap and give her heart to Jacob.

Returning to the theme of First Do No Harm, Briana gives up her negative attitude toward marriage after Jacob fulfills every promise he made, including changing his name to reflect her surname, and agrees to be married. The theme not only brought them to this moment by encouraging Briana to take a leap of faith, but it’s also a part of their wedding vows, illustrating the prime directive of their medical oath and showing how deeply dedicated they are to one another.
